Kindly be advised that as per regulations, the MBD/declaration forms are compulsory to all suppliers that are submiting for quotations/RFQ's at the City of Joburg. The following forms must be filled in full and be signed: MBD 4 MBD 6.1 MBD 6.3 MBD 6.11 These forms are obtainable at the following places: 158 Loveday Street, Braamfontein, Metro Centre, Block A, 15th floor, (Ask for Ricardo) NB: Failure to submit the above forms will lead to automatic disqualification of the submitted bid
